Group Rental Contract

Group Rental Contract

The group rental contract is required for all groups. Please download the file and fax the completed form into our office.

Waiver Release Form

Please copy this page for each person attending your event. Bethel Park REQUIRES a release form from each person attending events with overnight accommodations being provided. Please provide these for each member and present them to the BP director upon arrival. Parents or legal guardians must sign on behalf of any child under the age of eighteen.
